Here is a template for the Medication List. Follow the table and be certain to review the bullet points below in the document. This will help you start to look critically at your medications and ask questions to your doctors and other prescribers, and of course the pharmacist too.

The Question List is geared toward having meaningful questions to ask the health care provider. The list is split up into various scenarios such as questions for the doctor’s office, if affordability is an issue, when hospitalized, or when being discharged. This list is not exhaustive but will be a great start. There is space for your own questions too.
